Everyone is required to complete this form by September 11.

This form includes all of the scheduled rehearsal dates for the production. Out of respect for everyone's time, cast members will only be called to rehearsals when their presence is needed. In order for us to make that possible, we need accurate information about everyone's availability when we create the schedule. Please complete this form by September 11, even if you currently have no rehearsal conflicts.

I understand that the availability I provide on this form will be used to create the DTA winter show rehearsal schedule.

I understand that I should consider the dates I indicate as available to be a commitment to attend.

I agree to notify DTA as soon as possible if my availability changes, and whenever possible, to provide at least 2 weeks' notice of any new conflict.
